Through B’more for Healthy Babies (BHB), Baltimore City brings together communities, organizations, and resources so every baby might have the best start possible. As health care providers are trusted advisors for growing families, it is essential for them to be aware of the variety of resources available to patients and clients. This portal ensures the information providers need is only a click away. 

 

Hospital administrators and clinic managers play a key role in ensuring providers know about, and connect families to, resources in Baltimore City. By steering new and long-serving providers to this portal and ensuring your clinic or health care facility follows best practices, you can help improve generational health outcomes for babies and families across our city. 

Among the steps you can take to support this important effort:

​ENSURE PROVIDERS REFER PATIENTS AND CLIENTS TO ENHANCED CARE COORDINATION, which is run by HealthCare Access Maryland (HCAM) in Baltimore City. Referrals can open the door to available community resources including home visiting, WIC, breastfeeding support, Moms Clubs, behavioral health care, and more. (Also, these forms are required by Maryland law!)

  • For pregnant patients, providers should fill out the Maryland Prenatal Risk Assessment (PRA) at the first prenatal visit, or as a family’s risk factors change. This referral connects them to the range of resources available during and after pregnancy, offering support and relief at a critical time. 

  • Hospitals should fill out the Postpartum Infant and Maternal Referral (PIMR) before a family is discharged after delivery, connecting them to the support and resources — such as cribs, health care, and peer connections — that can help improve immediate and long-term outcomes. 

  • Direct referrals to Health Care Access Maryland (HCAM) can also ensure families find the services, resources, and insurance coverage they need to thrive. (Providers do not have to do it all on their own!)

​​ENSURE PROVIDERS ARE AWARE OF BEST PRACTICES FOR INFANT SAFE SLEEP, including by taking our video training and reviewing the range of resources aimed at supporting their practice — all available on the provider Safe Sleep page.  

​​REVIEW THE RESOURCES AVAILABLE THROUGH THIS PORTAL so you can help direct providers to the information they need to support positive health outcomes.

 

  • Review the data BHB makes publicly available on this portal. Annual birthing hospital profiles are also available upon request.
