EARLY CHILDHOOD PROVIDERS

The vital advice and resources you offer as an early childhood provider help every member of a growing family to thrive. B’more for Healthy Babies (BHB) aims to provide resources that you need, and some that you can share with families, to effect the best health outcomes possible. 


These one-pagers and the information below provides a quick review of what may be included during well-child visits to support your clients or patients — and your practice.​

CONNECT FAMILIES TO HealthCare Access Maryland (HCAM), which offers enhanced care coordination in Baltimore City. By linking patients or clients to HCAM, you help open the door to available community resources including home visiting, WIC, breastfeeding support, Moms Clubs, behavioral health care, early intervention through the Baltimore Infant and Toddlers Program, and more.

HELP THE FAMILY WELCOME AND CARE FOR THEIR BABY. This may include:​

​

  • Confirming a safe sleep plan for infants. The Safe Sleep page of this portal provides more information on safe sleep to support your practice and your patients. 

  • Talking about plans for — or challenges with — breastfeeding. Consider using this BHB resource to facilitate the conversation or educate parents.   

  • Talking about accessing WIC. Care coordination through HCAM can help ensure families have access to nutritious food. You can also directly refer families to WIC.  

  • ​​Talking about parenting and stress. Point parents and caregivers to the Family Tree’s 24-hour Parenting HelpLine (800-243-7337), a free and confidential service that provides advice, community resources, and support.

​​​​SCREEN FOR DEVELOPMENTAL DELAYS AT EVERY VISIT, and refer families in need of support to the Baltimore Infant and Toddlers Program

​​FOLLOW THE Maryland Healthy Kids Preventive Health Schedule for well-child visits. This includes family health histories, physical exams, risk assessments, laboratory tests, vaccine schedules, health education, and more.

 

​​CONSIDER WHAT THE FAMILY MAY NEED in coming weeks or months
 

​SCREEN FOR SUBSTANCE USE, MENTAL HEALTH, AND INTIMATE PARTNER VIOLENCE within the family

​​GUIDE FAMILIES TO RESOURCES if there is an infant loss

​

  • Roberta’s House offers support to parents who have experienced fetal or infant loss. Programs including Healing Ourselves through Peer Empowerment — the H.O.P.E. Project — can be accessed directly or through HCAM.
