From the very first prenatal visit (or before), you earn a family’s trust and support their health across generations. B’more for Healthy Babies aims to provide resources that you need, and some that you can share with families, to effect the best health outcomes possible.

This one-pager and the information below provides a quick review of what may be included during prenatal visits to support your clients or patients — and your practice.
COMPLETE THE MARYLAND PRENATAL RISK ASSESSMENT (PRA) to help patients access enhanced care coordination during and after the pregnancy. The PRA, required for patients on medical assistance, supports your practice by opening doors for patients and connecting them to enhanced care coordination, which is provided by Health Care Access Maryland (HCAM) in Baltimore City. Available community resources including home visiting, WIC, breastfeeding support, Moms Clubs, behavioral health care, and more.
SCREEN FOR SUBSTANCE ABUSE AND MENTAL HEALTH once every trimester. More information and community-based resources can be found on our Provider Resources page.
HELP PREPARE THE FAMILY FOR A NEWBORN. This may include:

Confirming a safe sleep plan. The Safe Sleep page of this portal provides more information on safe sleep to support your practice and your patients.

Talking about plans for breastfeeding. Consider using this BHB resource to facilitate the conversation or educate parents.

Talking about accessing WIC. Care coordination through HCAM can help ensure families have access to nutritious food. You can also directly refer families to WIC.

Talking about parenting and stress. Point parents and caregivers to the Family Tree’s 24-hour Parenting HelpLine (800-243-7337), a free and confidential service that provides advice, community resources, and support.
​GUIDE FAMILIES TO RESOURCES if there is a miscarriage or fetal loss.

Roberta’s House offers support to parents who have experienced fetal or infant loss. Programs including Healing Ourselves through Peer Empowerment — the H.O.P.E. Project — can be accessed directly or through HCAM.
